Medical Assistant Care Coordinator. This position is
Full-time: Monday - Friday 8:00am - 5:00pm supporting all of our service areas. Must be a graduate of Medical Assistant Program. (CMA preferred). The Medical Assistant (CMA) Care Coordinator works directly with the Practice Manager and the Case Management staff or Home-based Care team to provide high-quality support for patients receiving home-based primary care, palliative care, chronic care management (CCM), and other care models. This includes face-to-face visits, virtual visits, telephonic assessments, and phlebotomy services.
This role supports continuity of care by verifying continued clinical need, scheduling timely follow-ups, and addressing the physical, psychological, emotional, and spiritual needs of individuals with life-limiting or chronic conditions.
This position is responsible for contributing to patient care through assessment, intervention, care planning, care coordination, and documentation. The CMA care coordinator plays a key role in care transitions, program development, efficiency in front office workflows, and communication across healthcare teams. Requirements Education and Experience : - Graduate of a Medical Assistant Program (CMA preferred).
- Certified or trained in phlebotomy.
- Minimum of 1 year of clinical experience; 2 years preferred in post-acute, hospice, long-term care, or home health.
- Experience with Chronic Care Management or Population Health, or Primary Care preferred.
- Proficiency with Microsoft Word, Excel, and Athena strongly preferred
- Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
